An Advance Care Directive (ACD) empowers you to make clear legal arrangements for your future healthcare, end of life, preferred living arrangements and other personal matters. It allows you to appoint a Substitute Decision Maker to make these decisions on your behalf if you are unable to make them yourself.
Advance Care Directive Peer-led Workshops
Free resources and support are available in the western Adelaide region to help residents understand how to complete an Advance Care Directive. These peer‑led information sessions focus on building awareness, sharing experiences, and providing guidance in a welcoming environment, supported by interactive tools and trained facilitators.
*Please note that workshops are not for witnessing completed Advance Care Directive forms. If you need a Justice of the Peace (JP) to witness your documents, please refer to the Justice of the Peace availability calendar.
Advance Care Directive Kits
Did you know that you can now collect Advance Care Directive Kits, which include a step-by-step guide, at all five of our Charles Sturt Libraries branches? This includes Ngutungka Henley, Ngutungka West Lakes, Findon Library, Hindmarsh Library, and Woodville (Civic) Library.
These kits are popular and limited, so we recommend calling Charles Sturt Libraries on 8408 1333 on the day you plan to collect, to check their availability. When you arrive, ask staff for a copy.
