Advance Care Directive Peer-led Workshop

Do you have an Advance Care Directive or is it still on your to-do list?

An Advance Care Directive (ACD) empowers you to make clear legal arrangements for your future healthcare, end of life, preferred living arrangements and other personal matters. It allows you to appoint a Substitute Decision Maker to make these decisions on your behalf if you are unable to make them yourself.

We are providing free resources and supporting residents understand how to complete an Advance Care Directive through free information sessions. These workshops are based around sharing awareness, experiences and information in a welcoming environment with interactive resources with facilitators.

*Please note that workshops are not for witnessing completed Advance Care Directive forms. Kindly contact our Project Officer if you need to book a JP to witness forms.

ACD for Culturally and Linguistically Diverse (CALD) communities

We are actively working with our Culturally and Linguistically Diverse (CALD) groups in promoting community awareness and increasing completion of Advance Care Directives. We offer tailored workshops to CALD communities making information delivery suitable to group needs, including the availability of resources in different languages.

Volunteer Opportunity

If you have completed an Advance Care Directive and is passionate about helping others plan ahead, then you may be interested in becoming one of our Advance Care Directive Volunteers. The role aims to provide meaningful opportunities for volunteers to utilise their knowledge about ACD and support others understand how to complete the form by offering general information, sharing available resources, and providing support during workshops.

Key Activities:

  • Deliver ACD workshops in person using PowerPoint presentation or actively supporting workshop attendees (e.g. table discussions)
  • Build a rapport with workshop attendees
  • Be willing and able to travel to various locations within the City of Charles Sturt Council, Port Adelaide Enfield Council and City of West Torrens Council
  • Commit to and support at least one workshop a month with times and dates to be advised with at least four weeks’ notice where possible
  • Be willing to cover ad hoc volunteer shifts if necessary
  • Collect and forward relevant data and participation feedback to Program Team.

Essential skills, knowledge, and personal qualities:

  • Have completed their own Advanced Care Directive
  • Passionate about helping others
  • Ability to be reliable and flexible and work as part of a team
  • Good organisational skills and attention to detail
  • Ability to maintain confidentiality
  • Ability to communicate with people of all cultures, abilities, and backgrounds empathetically and respectfully
  • Basic knowledge with Microsoft PowerPoint or willingness to learn

Project Background

The Western Adelaide Councils of Charles Sturt, Port Adelaide Enfield, and West Torrens, supported by Office for Ageing Well and Health Services Programs, SA Health, will be supporting community members to complete their Advance Care Directive through Peer-led Workshops. These workshops will develop, deliver, and coordinate a peer-led and sustainable training model that aim to increase community understanding and completion of Advance Care Directives.
