Registration agreement

By ticking the box below you and your organisation are registering our involvement in the School Holiday Sports program, our organisation understands:

• It will conduct a free supervised physical activity session(s) at our club venue or suitable alternative.
• The program will cater for school-aged children OR younger children subject to negotiation.
• It will provide appropriately experienced and/or qualified persons who hold a current Working with Children Check.
• It will conduct activity in a safe manner and adhere to reasonable ratios of coaches to children to ensure maximum activity time for all participants.
• Ensure all activities adhere to any prevailing COVID-19 related guidelines and protocols.
• It will provide copies of its Certificate of Incorporation and Public Liability Insurance.
