Record of Visits and Reimbursements for Community Visitors and Neighbourhood Volunteers

This form can be used by current CVS and NVS volunteers only in order to lodge their record of visits to residents in Nursing Homes (CVS) and in the resident’s own home (NVS). This information is a requirement of them being a volunteer in these programs as the Coordinator must report visits and hours to the Federal Government.

What are the costs?

Reimbursements can be paid to volunteers if they choose – up to a maximum of $7.00 per visit.

What do you need to know?

You will need to supply the following information with your application:

  • Name of Volunteer
  • Name of Resident
  • Name of Facility they visit
  • If claiming reimbursement, we need to have either type of vehicle they drive and km’s or receipts such as bus tickets or cab receipts
  • Dates and amount of time spent with the resident.

How to apply?

  1. Complete the Application form.

What happens next?

Once the reimbursement form has been received, the Coordinator processes a payment for the volunteer (if requested) or simply records number of visits and hours.
